  4. Whereas, Thomas Gregory is the parent and guardian of the
  5. following child(ren): 
  6. April Mae, Franklin George 
  7. I designate Kenneth Wilson of Hanover, Maine to act as guardian of
  8. the minor child(ren) stated above upon my incapacity to so act. 
  9.  
  10. Should Kenneth Wilson be unable or unwilling to serve, I nominate
  11. Candy Flemyng of  Jupiter, Florida to act as the guardian of the
  12. minor children in the place of Kenneth Wilson. 
  13.  
  14. Upon my disability, the designated guardian shall have the
  15. following authority: 
  16.  
  17. a) residential custody of the minor child(ren); 
  18.  
  19. b) to approve medical treatment of any kind or type or to
  20. disapprove the same within the bounds of the law; 
  21.  
  22. c) to designate schooling for the minor children, and access to
  23. any and all of their educational records; 
  24.  
  25. d) to generally act in loco parentis. 
  26.  
  27. In the event that I am the custodian of any property for the minor
  28. children under the Uniform Transfer to Minor's Act, or the Uniform
  29. Gifts to Minors Act or similar statute,  I designate the guardian
  30. or successor guardian to act as custodian for all such custodial
  31. property.

  33. In the event that formal legal proceedings are commenced to
  34. establish a guardian for the child, it is my desire that the
  35. guardians mentioned herein have priority in appointment. 
  36.  
  37. The failure to list an individual as a guardian or successor
  38. guardian is intentional.
